Nursing Home Transition & Diversion Waiver (NHTD)
The NHTD program is a New York State Medicaid Waiver program that offers individuals with disabilities and seniors who are eligible for nursing home placement to receive services in their home and community. This option allows for a greater quality of care in the least restrictive setting possible. Seniors will receive appropriate homecare services permitting them to live in the most appropriate, least restrictive setting, which is usually their homes rather than an institution.
The NHTD program is targeted at individuals either already in nursing homes and wanting to return to their community, or those being considered for nursing home placement but who would prefer to avoid institutional care and remain in their own homes.
Eligibility and Referrals
Must be an active Medicaid recipient.
Must be screened and eligible for a nursing home level of care. The NHTD Waiver Program is a choice-based model; however, referrals must first be made to the Fulton or Montgomery County (dependent where the patient lives). Covering Fulton, Montgomery, and Saratoga County.
