How do I obtain home care services for my loved one?

It's very simple! Just give CHC a call.

We will contact your doctor and help work out the details. You can also ask your physician to refer yourself or a loved one to Community Health Center.

Either way, services will soon be coordinated between CHC and the doctor for the most comprehensive home care available in the 7 counties we serve.


Who oversees my health while CHC caregivers are taking care of me?

The majority of CHC services are overseen by a nursing supervisor, under your doctor’s plan of care.

CHC is like an extension of your physician’s office, as they approve all care given to you by CHC.


How can I pay for CHC services?

CHC home care can be paid for by Medicaid, Medicare, HMO’s, private insurance and private pay.
