Q: How can I pay for CHC services?

CHC home care can be paid for by Medicaid, Medicare, HMO’s, private insurance and private pay.

CHC can help you wade through the details of payment, and help you discover options for payment. Just give us a call!

Q: Who oversees my health while CHC
caregivers are taking care of me?

The majority of CHC services are overseen by a nursing supervisor, under your doctor’s plan of care.

CHC is like an extension of your physician’s office,
as they approve all care given to you by CHC.

Q: How do I obtain home care services for my loved one?

It’s very simple! Just give CHC a call,
and we can take it from there.

We will contact your doctor and help work
out the details. You can also ask your physician
to refer yourself or a loved one to
Community Health Center, and let them make the call.

Either way, services will soon be coordinated
between CHC and the doctor for the most
comprehensive home care available in
Fulton or Montgomery counties.
