CHC clinicians (RN’s, LPN’s, Therapists, MSW's, RD's, and others) have the wonderful chance to utilize their skills on a vast range of patients with many different challenges.

Rather than work in an institution where they are responsible for several patients at the same time, CHC clinicians get to interact with their patients one-on-one, and help directly influence the care plan created by the physician overseeing the case.

Positions Currently Available

Supervising Community Health Nurse (SCHN)
Full-time position – for qualified RN to supervise the clinical staff; is responsible for staff performance, adherence to policies and regulations and patient care outcomes.
Current RN license, Bachelor’s degree in nursing or health or human services field and two years’ experience in home care.

RN Case Manager
Full-time position for a RN to provide home health nursing and case management to patients in Fulton/Montgomery Counties. Current RN license required and one year nursing experience and BLS preferred.

 Certified Home Health Aide
Positions available in Fulton/Montgomery Counties for enthusiastic individuals to provide positive one-on-one care to clients in their home. HS Diploma or GED preferred. Current HHA certificate, driver license and reliable vehicle required.

Housekeeper
To provide routine housekeeping work for patient’s in their home. HS Diploma or GED preferred, driver license and reliable vehicle required.
