We’ve all heard the phrase, “home is where the heart is.” When we are sick, or injured, the most comfortable and welcoming place is home. Thanks to Community Health Center, our loved ones can stay at home rather than enter a nursing home, and patients who have just left the hospital can recuperate or rehabilitate in the comfort of their own surroundings.

Community Health Center serves more than 525 patients each week in Fulton and Montgomery counties with its team of 48 professional nurses. The facility has a total staff of 160, that includes nurses, home health aides and housekeepers, as well as physical, occupational, speech, respiratory therapists, dietitians and social workers. Professionals provide these services to patients who have just gotten out of the hospital, to those who need long term daily care or assistance, or to children from birth to 5 years old that need developmental services. The company has been providing health care to area residents since 1985.

There are many ways that CHC could be used to help you or someone you love receive comprehensive care at home. CHC nurses, aides, therapists and specialists work only under the supervision of the patient’s doctor, and have direct contact with the physician to help him or her manage your care.

CHC can visit on a long-term basis, for those who are chronically ill, or aging, or on a short-term basis for someone who has been injured or is transitioning to home after a hospital stay.

Our nurses are first rate, and our entire business culture is focused on one simple thing: Patient Care. We provide our patients with education about their condition … training for wound care or managing diabetes … physical therapy … or any type of skilled nursing.

We are available to the patient 24 hours a day and 7 days a week.

For patients who need continuing care, we offer a long-term home healthcare program. Patients include the elderly, chronically ill, and disabled who need ongoing health services. This service is also known as a “Nursing Home Without Walls,” because in many cases, our patients are able to avoid going into an institution thanks to the care of their nurses and physicians who work with them at home.
