A Word from CEO Paula Reichel

Welcome to Community Health Center Online, and thanks for taking the time to visit us!

Home care is one of my passions, after spending many years in a hospital setting, treating many of the very patients we care for today in Fulton and Montgomery counties. What I found out 10 years ago when I came to home care was amazing. Home care professionals are loving, generous people who give so much to their patients every day, and many will go above and beyond the call of duty to help those they care for. In some instances, CHC staff members are the only “family” our patients have, and we do not take that responsibility lightly.

Our agency philosophy revolves around one idea -- that OUR PATIENTS and families are the center of everything we do. We love them, and they love us! For that we are grateful, and we promise to continue to provide care as a labor of love and compassion in our community.

Values & Mission

Our Goal: "To Bring Health Care ... Home"

     You can get a great sense of CHC’s values as an agency by watching one of our caregivers enter a patient’s home. There is always a polite knock at the door, then a smile, a warm greeting, and a respectful dialogue about how the resident is doing as a patient – and as a person.

      It does not matter if the employee is a home health aide there to help a patient bathe and get dressed for the day, or a Registered Nurse is visiting to administer IV antibiotics or treat a complex wound. Each CHC customer is treated with professionalism and respect, as well as a kind heart.

     Our staff members are also treated well by the agency. They are employed in a supportive environment where professional growth is encouraged. We believe that if a caregiver is not supported on the job, they cannot extend that caring spirit to our patients.

     Our nurses, therapists, home health aides, housekeepers and other caregivers are first rate, and our entire business culture is focused on one simple thing: Patient Care. We provide our patients with education about their condition … training for wound care or managing diabetes … physical therapy … or any type of skilled nursing.

promote and support the provision of the appropriate services which will enable individuals to maintain their independence in their homes or in community settings.

VISION


      The Vision of “Your Home Care Network” is: Within the Northeast Region of New York State, Your Home Care Network will promote and support a full continuum of quality and leading edge services which will respond to the developing needs of the community.
      The member organizations will be known for their stable, competent and respectful workforce. We will promote and support our services in a fiscally responsible manner, either through integration within our network or through collaboration with other providers.

      Values represent the core priorities in the organization’s
culture, including what drives members’ priorities.
